Health
When planning your journey speak with your loved one’s doctor or their carers. They will be able to discuss any concerns you have over their health. You may need to make sure that they continue to take their medication despite the change in their routine.
You should also be carrying a car first aid kit with extra supplies such as bandages, antibacterial wipes, tissues, and aspirin.
Keeping everyone hydrated and fed is also important. Have water and snacks handy.

Safety
Take some time to make sure they are strapped in. A seatbelt may become uncomfortable for them so you may want to invest in a seatbelt pad.

Think of some travel games that you can play along the way to keep everyone entertained. You could also prepare some of their favorite music to play.
Mobility
If they have limited mobility, then you may want to get a handicapped permit which will allow you to park close to wherever you stop.
You may also want to bring a wheelchair so that they don’t have to walk too far at any given time. If you don’t have much room, you could look at mounting the wheelchair to the back of your car.
